If you would like to cancel your membership of the Central Bank of Ireland Income Protection Plan or Central Bank of Ireland Group Life Plan, please complete an opt-out form below. If you opt-out before the end of your 4-month free period, any premium you have paid will be refunded to you. It is important to note that if you cancel your cover and decide to join later, you will need to complete an application form and undergo full medical underwriting.
